  3. Copy of national passport
  4. Certificate about secondary education (original) and notarized copy with translating into the Ukrainian language and legalization (in 2th copies)
  5. Original of diploma of junior specialist* with a supplement and notarized copy with translating into the Ukrainian language and legalization
  6. Original of diploma of bachelor** with a supplement and notarized copy with translating into the Ukrainian language and legalization
  7. Original of diploma of master's degree*** with a supplement and notarized copy with translating into the Ukrainian language and legalization
  8. Document about completion of preparatory department (at presence of)
  9. Policy of medical insurance, if other is not foreseen by the international agreements of Ukraine
  10. Insurance from cause of deportation
  11. 6 photos of 3х4 and 4 photos of 3,5х4, 5 (for registration of temporary residence permit in Ukraine)
  12. Consent to personal data collection and processing and completed questionnaire
