1) The original invitation for study or the original transfer letter (signed by the Vice-Rector of another higher educational institution of Ukraine, at the invitation of which the foreign citizen arrived to study in Ukraine); 2) Questionnaire of the established sample (to be filled in at the university); 3) Application of the established sample (to be filled in by the university); 4) A valid foreign passport with a visa type "D" and a photocopy of all completed pages of the passport in 3 copies; 5) Notarized translation of the passport into Ukrainian into 2 copies (originals and 3 photocopies); 6) Original document on complete general secondary education or basic higher education, as well as annexes to the educational document containing information on academic achievement; 7) Notarized translation of the educational document and its supplement in 2 copies (original and photocopy); 8) In the presence of a certificate of completion of the preparatory department for foreign nationals of higher educational institutions of Ukraine and a photocopy; 9) The original of the birth certificate and its notarized translation (original and photocopy); 10) A medical certificate of health status, certified by the official health authority of the country from which the foreigner arrived and was issued no later than two months before the date of entry to study in Ukraine; 11) A medical certificate on absence of HIV infection and AIDS, certified by the official health authority of the country from which the foreigner came and was issued no later than two months before the date of entry to study in Ukraine; 12) Agreement of voluntary health insurance of foreigners temporarily staying in the territory of Ukraine; 13) Contract of liability insurance for reimbursement of expenses related to the execution of the decision on expulsion of a foreign citizen from the territory of Ukraine; 14) 12 photos 35 x 45 mm size. We emphasize that in order to enter the institution of higher education, a foreign document on education must be legalized and certified in the manner prescribed by law. Appendix to the document on education (if available) have to include information about form and term of training, the list of educational disciplines (subjects), the general (weekly) volume of classroom and independent hours (credits), quantity of semesters and other components of educational process is specified. Attention! The type of legalization (document containing the Apostille stamp or consular legalization, except for countries with which Ukraine has bilateral or multilateral agreements containing provisions on the recognition of documents without additional certification (without legalization)) depends on the country where the educational institution issued educational document.
